By filling out this form you are helping us build a network connection map. This will better enable us to establish a relay system so that we can communicate across the city, across the country and around the world.

When you make a connection with a like-minded individual, one of you should fill-out this form as best you can. Note the same fields are there for both sides of the connection - ie Node 1 and Node 2.

We will then use that information to build our network map.
